Please plan to arrive at our office 15 minutes before your appointment time. We ask that you complete the Medical Questionnaire detailing your past medical history. This is available on the patient portal. It is important this information be completed prior to your visit. If you are a male between the ages of 45 and 75 or have low testosterone, erectile dysfunction or have Peyronie’s disease please also complete the AUA Symptom Score, and the ADAM and SHIM questionnaires. Bring any test reports or films pertaining to your medical condition with you to your appointment.

You will need to present your insurance card and ID at check-in and will be asked to review and sign our financial and policy. You should check with your insurance company to see if they participate with our practice or if you need a referral to be seen by our providers. You will need to pay your co-pay at the time of service. Your co-pay amount should be listed on your insurance card. We accept Mastercard, VISA, Discover, and American Express, cash or personal checks. If necessary, we provide financial counseling services.

Finally, please be prepared to give a urine specimen after you arrive for your appointment. If you need to use the lavatory before being called, please speak with the receptionist.
